Application Process
Requirements for Admission Applicants for admission to Simmons Institute must have at least a high school diploma or a state approved equivalency certificate. Applicants must also meet the requirements demanded by the state in which he or she intends to be licensed. Application for Admission Form A person wishing to enroll in the program at Simmons Institute of Funeral Service must complete the Application for Admission and submit it to the Registrar's Office , along with a nonrefundable $50 application fee. Additional Application Requirements In addition to the above Application for Admission, the applicant is also responsible for submitting the following: A brief typwritten essay about him or herself and why he or she is interested in funeral service as a career. A copy of his or her birth certificate. If there has been a name change, a copy of the appropriate documents verifying the change (i.e. marriage license). An official copy of all high school transcripts, sent directly to Simmons Institute from the school, or equivalent. Official copies of any college transcripts, sent directly to Simmons Institute from the college, if applicable. The signed Track Record/Campus Security Disclosure (provided with the Application Form). Report of Separation from Active Duty (DD-214-long form) if military time has been served by the applicant. The completed Medical Information Form, signed by a licensed physician. (Form provided with Application materials.) Personal Interview In addition, a personal interview with the administration of Simmons Institute is part of the application process, unless waived by the administration due to extenuating circumstances.
